In order to save a filled in application you must have Acrobat 9 installed on your computer. Adobe Reader is provided as well if necessary, however Adobe Reader will not allow you to save the form after it is filled out. It will only save blank copies. If you are unable to save a filled out version of the application, please print and fax to Michelle D Fraley at 859-239-6742. Applicants still need to submit hardcopies as instructed in application directions.
