Heart Failure Education Program
If you have been diagnosed with congestive
heart failure, your quality of life may be greatly improved
by participating in our Heart Failure Education Program.
Congestive heart failure is the number one reason for
adults age 65 and older to be hospitalized. The condition
develops when the heart's muscle becomes weakened due
to illness or a chronic medical problem. As a result,
the heart loses its ability to pump enough blood to
supply the body's needs.
Our Heart Failure Education Program offers comprehensive
disease management for patients with congestive heart
failure as well as cardiomyopathy and chronic ischemic
heart disease. The goal of the program is to assist
you in managing your disease so that you can enjoy a
better quality of life.
The program is coordinated by an advanced nurse practitioner
who has completed specialized training in the care of
individuals with heart disease. The program uses a multidisciplinary
approach for managing heart failure in patients. The
multidisciplinary team works in conjunction with your
referring physician to develop an individualized plan
for you. Services include:
- frequent follow-up on patients
- support to patients and their families
- patient education
- assistance with medical prescriptions
